Check your insurance directory for in-network providers as your benefits will be substantially different if we are an out-of-network provider. You can call us with any specific insurance questions you may have.

What will I need to bring to my first appointment?

• For your first appointment or if you are an existing patient that has not had an appointment in over a year; you will need to bring your insurance card, your driver’s license, a list of all current medications, and we will need to take your picture when you check-in. As a new patient or an existing patient with a new complaint, you will need to fill out the new patient forms. This may be done through our website (here), or may be printed and brought to your appointment.

• Dr. Smith operates surgically at Quail Creek Surgical Hospital. This out-patient facility is partnered with BSA Health System.

• No, our office offers a variety of treatment options including physical therapy , injections and surgery. Dr. Smith will talk with you and decide which route is best for your problem and your needs.
